Municipalities with CTP primary weight corridors will need to complete a statutory declaration in order to receive the $1000 per kilometer maintenance assistance for the year 2011-12. The Statutory declaration form can be found on the SARM website by following the link: https://www.sarm.ca/services/MREP.
